Diabetes Champions

Diabetes champions give their time on a voluntary basis and deliver their health messages in various settings including churches, mosques, community centres, supermarkets and fun days in parks. They are primarily focused on signposting people with a potential risk of Type 2 diabetes, identified via a diabetes risk assessment process.

Champions undergo a considerable amount of diabetes training, and are recruited from all socio-economic classes and ethnicities. Information is provided on what is diabetes, nutrition, annual checks including the foot, and NHS Health Checks.

Diabetes Resources – Know Diabetes

The Know Diabetes website is for people with diabetes living in Hammersmith and Fulham, Kensington and Chelsea, Westminster, Ealing, Hounslow, Brent, Hillingdon and Harrow.

It is the first place to look for all your diabetes needs as it is interactive and houses over 200 resources – videos, games, online courses and leaflets to help you self-manage your diabetes. There are specific resources for children and young people, minority ethnicities and those with limited english, and it provides details of support groups and local services.

Diabetes Services: foot checks, eye checks, diabetes teams

The Know Diabetes website has a search tool that helps you find all the local diabetes services in your area, including: Hospital services, community diabetes team, podiatry, retinal screening, health checks, health trainers and diabetes nurses.

Diabetes Support Groups

If you have diabetes and want more information, or simply want to share your experiences with those in a similar situation, you can attend a group in Hammersmith and Fulham, which is affiliated with Diabetes UK and sponsored by Central London Community Healthcare NHS Trust.

Diabetes Education

Everyone who has been diagnosed with diabetes should be offered a chance to attend an education course. This is best done around the time of diagnosis if possible. These courses are called ‘Structured Education’. This site gives information about types of structured education, both via online courses and in groups.

Diabetes Prevention

Your local Healthier You: NHS Diabetes Prevention Programme is here to support people at risk of developing Type 2 diabetes. Programmes will be held in a range of locations across Westminster, Hammersmith & Fulham, Kensington & Chelsea, Ealing, Hounslow, Brent, Harrow and Hillingdon. If there isn’t a programme running in your area at the time of your referral, you will be placed onto the area waiting list until the next programme begins. You also have the opportunity to attend programmes in other areas should you wish.

You can register for the program online, without referral, if you have your NHS number, your GP details and the results of a blood test within the past year that shows you are at risk of Type 2 Diabetes (HbA1c 42-47.9 or fasting glucose 5-5-6.9).
