The Expert Patient Programme

This is a Self-Management Programme to help people living with a long-term condition gain greater self-confidence in their ability to manage their symptoms and how their health problems affect their lives. It is a series of highly interactive workshops, delivered once a week over six weeks, in a community setting.

The Programme is FREE to anyone living with a long-term condition in Hammersmith & Fulham, Kensington & Chelsea or Westminster.

Support for self-care

Self Management UK

On average people with long-term conditions spend less than 3 hours a year with their health team. The rest of the time they are left to self-manage. Our charity exists to help these people be understood, empowered and supported.

Self care Forum

The Self Care Forum aims to further the reach of self care and embed it into everyday life. Self Care is the actions that individuals take for themselves, on behalf of and with others in order to develop, protect, maintain and improve their health, wellbeing or wellness.
