CLCH: District Nursing

District Nursing provides nursing assessments and care for housebound patients with a physical healthcare need. Nurses see patients at home and in residential care settings. Patients must be registered with a GP to be eligible and need to be referred by a GP, consultant, other health provider or social services. At night, referrals are via the NHS 111 service.
